Jim,
High level combining is very commonly done with solid-state
power amplifier modules. If you search the app-note archives
on the On Semiconductor website, you can find copies of the
old Motorola Applications notes that Helge Grandberg authored
on power combining techniques. If you have any trouble finding
them, drop me a line. I have electronic copies at work which I
can email you.
I believe both RF Parts and Communications Concepts sell
parts for building these sorts of parts.
