Below are the promotion details from the Staples Web Site.
Note the part that says "One rebate per name/family/address."
The rebate processing centers are all computerized, and will
exploit every opportunity to deny a rebate. If you submit
more than one rebate with the same name or address, it will
almost certainly be rejected. You won't even be notified. They
just won't pay.
Rebate applies to Select Envision Monitors Model #EN 710 (SKU 450426) or
Model #EN 7100 (SKU 466430) purchased at Staples stores, Staples.com or
Staples Direct only. Purchase must be made between January 5, 2003 and
January 11, 2003. All requests MUST be postmarked by February 11, 2003.
Offer not valid with any other rebates, offers or promotions. Offer valid
in US only. Good for actual purchases by end users; excludes resellers
and distributors of products and their families. Please allow 8 weeks
after claim is received by fulfillment company to receive your rebate.
All rebates will be made in US dollars. Manufacturer is not responsible
for lost or misdirected mail and illegible entries. Staples logo and
product names are trademarks of Staples and their respective companies.
One rebate per name/family/address. For questions about your rebate call
1-877-266-6483. Fraudulent submission of multiple requests could result
in federal prosecution under the U.S. Mail Fraud Statutes (18 USC,
Section 1341 & 1342). Keep a copy of your claim for future reference.
