Yes sir, it is HRPG-ASCA#17C made by Avago (Now a part of Agilent) and sold by 
Newark Electronics, UK.

The cost was less than $40.00 including shipping.

Do you know what the mfg part number of the encoder is? My guess is that it 
will be a common, 128 or 256 PPR optical encoder with a 1/4" shaft. You should 
be able to find a new replacement on ebay for less than $50.
