Thanks for the replies. I was told it is an older KLM model KT-34 tribander 
yagi. It definitely would need new replacement element insulators and other 
plastic pieces, so I think it will be relegated to the "spare aluminum tubing 
pile" unless I find someone locally who wants it. It would definitely need the 
M-Squared refurbish/upgrade kit and a boom, if it was to be put back in service 
as a KT-34. 

Thanks to Jim, W7RY and others who ID'ed it after seeing the pictures.
