I had a similar problem:
CQ on radio 1
line up QSO on radio 2, number 41 gets assigned, he works someone else
someone answers on radio 1, WL hands out #42
guy on radio 2 disappears, wipe QSO
guy answers on radio 1, WL hands out #43.
No one ever got #41
