SPECULATED CAUSE:
The FL 2100 has no grid current meter. If you tune up a GG amp for absolute
maximum power ouput, you often have a soaring grid current above spec,
unless you add the slight bit of extra loading, as recommended. The extra
loading drops the output slightly (insignificantly), but usually
significantly drops grid current.
I bet the 572B failures are due to very high grid currents that are not
metered, therefore unkown to the operator.
