Several plastics distributors have stores in many cities and can cut
and machine plastic to meet your need. They can also sell you the
cement you'd need if you should want to join pieces together. "Regal
Plastics" would be a good bet in the midwest. ":Austin Plastics" in
central Texas. There are probably others in your area. Most will
want a simple sketch.
