I used a Carling 610/620 series.  I think it's the same one TT used.  They are available at Mouser Electronics.  It's a dpdt and has word fine for a few years.  let me know if you have any questions.

Thanks for all the responses.  The nice people at Mouser electronics found a fan with the same specs.  We will see what it looks like when I get it.  I've replace one before and it's a bit of a job.
