Hans My 89' US Tower crankup has 2 anchor bolts at each leg, a total of 6 bolts. Each bolt is 1 -1/8" diameter and 27" long of ASTM grade 36 steel. I think I'd trust US Tower's licensed PE wet-stampe

Yep, because closely spaced bolts can more easily pull out a *chunk* of concrete containing all the bolts than widely spaced ones. I was thinking of bolts *slipping* out of the concrete. -Steve K8LX

I think that properly installed bolts in concrete, whether "cast in place" or chemical anchors held in by adhesive, fail by either the bolt pulling apart (metal failure) or the concrete pulling out.
