Probably a case of WAY too much headphone sensitivity. Many audio problems can be minimized by adjusting headphone sensitivity such that you have comfortable audio level in the 'phones with the volum

Is it intelligible (but just pitch shifted) or unintelligible? Does the pitch change with USB or LSB on the same band? I would guess it's one of: HF getting into the audio; SSB 9MHz leakage in the RF
