Those element insulators are from Hy-Gain (long ago). My best guess, per your description is a Hy-Gain TH-3 (very first one they came out with), but not sure why one element has no traps. Perhaps the

Tim, If you'll look at page 46 and 47 of this 1969 Andrew catalog (link below), it mentions the 900 series antenna and seems to match your description. I wouldn't think there would be a lot of value

Tim, what you're looking at is a DC grounded ground-plane antenna. Tunable from 25 MHz to the top end of Two Meters. I've seen these in 2-way radio systems for some of the school bus yards I service.
