Look at TDK CC45 capacitors.  Available at Mouser with number 
810-CC45xxxxxxxx.  They are about $.15 in 1k quantities.  The data sheet 
specifically says "low dissipation at high frequency".  They are rate at 
6kVDC.  I have tried*unsuccessfully* to blow them up at HF with no DC 
and RF voltages of 2kVpk.  However the ones I was testing were 10pF, 
22pF, and 47pF so the current would be much lower than the higher 
capacitance ones.
