I want to thank the six guys who responded to the query, below,
including K5DJ.

There were two of the scarlet B's, and it was not immediately apparent
why, as the log window entries looked fine---hence my request. But when
I created a Cabrillo file and studied it, the answer was apparent--the
operator had mistyped his entries, then apparently corrected them, but
the modes had gotten buggered. This did not appear in the log window
because the MODE column was not displayed. When the MODE column was
displayed, the errors were obvious, and quickly repaired.
