[AMPS] Henry 2KD-5 Top Cover Plate Needed

I got a Henry 2KD-5 at a good price without a top cover plate (how do they
lose these things???).  The opening measures approximately 12" x 16".  Does
anyone have one of these they could part with?  Name your price!
