A commercial HF amplifier manufacturer uses silicone insulated cable which 
is used in neon lighting applications.
Check this link: http://www.bambachcables.com.au/other.htm and search in 
this page for:  neon sign lighting discharge high tension cable
The cable is rated for 7.5-15 kV

Besides the HT voltage ratings I guess the current rating is important too. 
Silicone insulation might have an advantage in high temperature conditions. 
The SHV plugs are rated 10 amps, the  plug dielectric 5 kV and working 
voltage 3.5 kV rms so you I guess one could use the SHV plugs on a 20 kW 
output class AB1 amplifier
