I think another factor is the amount (or otherwise!) of cooling air over the 
filament pins. Other tubes using that filament (5v, 14.1A) have requirements 
for the amount of cooling air over those pins, and it's not always easy to 
determine if there is the correct amount of cooling air flowing.
I use a normal 'Boxer' 5 inch fan blowing directly on the filament pins (one 
for each tube) which is almost certainly over kill, but does seem to avoid the 
problem.
