[Amps] Modulation Transformer Info

Listmates,

Does anyone have connection and pinout info for the Thordarson T-21M62-A 
universal (multi-match) modulation transformer? I have plenty of "spec" 
data but no connection info. I think these transformers came with a 
detailed info sheet.
