*Sometimes*, the CQing operator can blame himself for this.

One comes to expect a certain "rhythm" to a contest CQ - "CQ TEST W9WI
W9WI", something similar.  If your CQ is unusually long or has something
else at the end, ("CQ TEST W9WI W9WI K") the calling operator may not expect
it & may launch his call before you're done.  I would suggest if you're
having frequent trouble with this you might want to take a look at how
you've got your CQ programmed.

But often, it simply is someone a bit too fast on the switch.
