What you said!

I called before my last submission and was told to follow the procedure
Steve gave below. I did it and I had no problem.

I think the one area that has been a little complicated is sending in cards
for credit at the same time as doing a LOTW submission. Seems I read that
this had been difficult for a few guys. I have stayed away from this. Maybe
it has all been resolved by now?

I find the folks at the DXCC desk to be friendly and helpful. Unfortunately
it is SLOW!! The ARRL Board of Directors has authorized the hiring of an
additional person to speed up the process. Should be getting better soon.

No, I Bill Moore will agree with Wayne. Here's what it says on the form:

"Note: Cards may be submitted directly to ARRL or checked by a DXCC Card
Checker. If cards are sent direct to ARRL, it is not necessary to fill
out this form. This form must be completed if a Card Checker checks the
application. In either case, the cards or listed credits must be sorted
first by band then by mode. If you fill out the form, supply all
information as requested. Be sure to use the Entity name, not just the
prefix. Cards indicating multiple contacts must be placed together. If
cards with multiple credits are submitted direct to ARRL, a notation
must be made on each card indicating which credits are to be entered.
If no indication is made on a card, all credits will be entered into
 your record. "

In other words, you have to sort your cards by band and then mode regardless
of whether you are sending them in to the DXCC desk directly or if you're
having them field checked. If you're sending your cards in to the DXCC desk,
it isn't necessary to fill out that form. If you're having them field
checkedd then you do have to fill out that form.

are very specific regarding applications for DXCC.... QSO data on your cards
is compared to the data you entered on your application. If you have
successfully done it your way, you must have a close friend on the DXCC desk
that's looking out for you. 73, Dallas W3PP
