Interestingly on my OMNI VI+ the factory installed encoder was an OAK-GRIGSBY 90Q125-02-00245 with a date code of 12/08/97. It has a similar pin configuration to the Bourns replacement that Curtis found, but is a different form factor.

I replaced the original encoder today with one of the Bourns' encoders from ebay and it is working very nicely. It is an easy replacement - four screws to take off the case and four screws to drop the front panel assembly then remove the old encoder and put in the new one. Very simple.

I thought I'd pass this along: My Omni VI wouldn't change frequency so I subbed 
the RIT encoder and confirmed a bad encoder. I found one on Ebay that not only 
was close but better than stock (it's got a ball-bearing shaft instead of the 
stock one with a bushing only). Not only is it working better, it's only $21 
shipped! Needless to say but I bought another one today to have on hand. Here
