Its the "A147-4" and its manual is on the Cushcraft/MFJ Web site. The combined 
manual covers several 2m, 220, and 70cm beams.

Yes. I had one. Four element two meter antenna, designed for the FM/ 
repeater portion of the band. The elements were mounted in the  
vertical plane.
I bought mine in 1974. No idea when it went out of production.
