1. It isn't a CQ sponsored contest, so the plaque should make it to the
winner ;^).
2. I've never seen the plaque but if the sponsor isn't engraved on it, all
it takes is a look at the website where plaque winners and sponsors are
listed.

The same station has probably won it a number of times and probably uses
them to shingle his roof.
