It doesn't have to be just an in band multi op lock out. Its really any lock out - in band or other band on MS. Or it can be SO2R and the run frequency picked up again. Based on the number of times it happens to me, its mostly MS stations for sure. I give them one shot and move on. They are always back in 1 - 10 Qs and they learn their lesson 100% of the time.
