If you have significant coupling from the driven element(s) to the coax,
then adding a common mode choke would change the radiation pattern of
the system. But adding the choke would not significantly increase the
loss IN the coax.

Only if you're using an end-fed antenna without a counterpoise. The
choke prevents the antenna from using the feedline as a counterpoise.
