As someone who has accomplished this without concern about any of the things in
your list, let me make a suggestion for what worked for me. One word: contests.
ARRL Sweepstakes
ARRL 160 contest
North American QSO Party
CQ WW 160
CQ WW WPX
These are just a few (I'm not a hard-core contester), but they are what got me
the states (and they're all confirmed on LotW).
