The Petzl Absorbica Y MGO lanyard is basically what you are after. I am a big
fan of Petzl when it comes to my safety. A google search for that product will
turn up various online vendors. It's about $80. You'll need a locking biner to
attach it to your harness.

available that fit your basic requirements.
http://www.msafetyproducts.com/1224409.htm is a direct link to the "Shockwave
2" lanyard which has two rebar hooks and a locking hook for attachment to your
harness.
