[AMPS] TUNER HELP NEEDED

Anyone have a manual for or familiar with the 

PALOMAR   ENGINEERS  ANTENNA   TUNER   PT-2500A?

I lost my manual and the tape with the connection information
has long since fallen off.

Playing with possible alternatives for balanced line has not
been successful.  I suspect either a damaged switch or
balun.
