IF you chase me next month (after I'm back from a business trip to the US and the XYL is back from her business trip to the US - she's in Portland Or at the moment ), I'll dig it out and get her to scan it on her computer and send it to you.
BUT you will have to chase me, bearing in mind that I have 2 business trips to France and one to Sweden in February - so far.
