My receipt, which goes back to January, 1997, lists the following address:
Lawrence Livermore National Laboratory
Attn: Gerald J. Burke, L-156
P.O. Box 5504
Livermore, CA 94550

an old e-mail address for Jerry Burke is given as Burke@icaen,llnl.gov..
A more recent address is burke2@llnl.gov. This will be a good route to
obtain the current pricing plus any other details of ordering.

Hope this helps.
