Speaking for the ARRL 160/10 logchecking team, penalties are not enacted by 
removing additional QSOs, but by removing an equivalent number of QSO 
points from the total QSO points.  The only mults which can be lost are 
from the penalized QSO itself, not the next one or three QSOs.

"Equivalent number of QSO points" is important in the ARRL 10M contest 
because CW QSOs are worth two points and SSB QSOs worth one point.  Bust a 
CW QSO you lose two QSO additional points, bust an SSB QSO lose one.

FWIW, in the discussions about the three QSO penalty, the ARRL penalty is 
one additional QSO, which is for a NIL.
