In this example, the other guy DID get a confirmation.

In this example, you DID get confirmation.

The QSO took place, you just messed up when you logged it.

I agree there should be a penalty (although I think 3 Qs is harsh), but 
there also should be a way to submit certain QSOs as check QSOs only.
