[Mldxcc] DXCC application questions

I need to submit both paper QSL's and LOTW credits for DXCC.
There seem to be at least three distinct processes:

1.  A paper application for QSL's
2.  An online application for QSL's
3.  A different online application for LOTW

#2 makes no mention of LOTW
#3 makes no mention of QSL's

Do I have to submit both #2 and #3 and pay
two application fees?

I vaguely remember applying for (I think) WAS
and I could do both LOTW and QSL's on the same
on line application.

Additional question:  Does "DXCC card checking" at
Visalia mean they do NOT do WAS card checking?  Or
do they do both?
