[Mldxcc] Cabrillo QSO Template

Greg,
I know the Cabrillo file from my program is not right, too. 
Maybe someone could send  a few lines from their log to the 
reflector so we can see exactly what goes where? 
Mine left off the state and the CA counties, and put in 599 for all 
the reports, which we didn't even use on this contest.
