I would like to add a couple of comments to Rick's point 2 -- rate.  During WRTC and for some multi's in the know, dueling CQ's were allowed on the same band.  This maximizes the transmit capabilities of the station so that there is almost always a transmitted signal being send out.  You might have noticed that this practice was banned, mainly due to claiming more frequencies by loud signals without adding participation.
 
However we can effectively achieve the benefits with a mult op who is watching closely to the rhythm of the run op even when he is running 200/hr.  We had some fun doing this on 10M this year in the phone contest where there was never a situation when the needed window was empty.  When the run station finished transmitting, the mult op had a new one ready and quickly sent NQ4I.  Sometimes the spacing did not allow the mult op to give the exchange but if you are late, it doesn't matter because the station you called will give your call and exchange a second time.  In these situations, it is better to select the loud, fast stations first.  Pre-programmed F4 and F2 sending at 35 - 40 wpm should do the trick.
 
In the CW test with skimmer, there will always a needed station for the mult op to work.  New, rare mults will require the mult station to step on the run guy but new, pedestian type mults can be worked with the same technique described above.  Of course, if the run op is having trouble with the weak ones, then interstation interference will reduce the opportunities however when the band is open, opportunities should abound.  Mult stations transmitting shouldn't be a zero sum game but 1 + 1 = 2!
