After reading your description of the antenna ... my first, and gut  reaction 
... was that is is/was Hy Gain's Model TH-5. I have a friend who had  one ... 
and used successfully for many years before going to a Tennadyne  Antenna. He 
indeed, was very happy with it.
 
My "guess" is that it's a TH-5. But ... I could be incorrect?
