I suspect the barrels. There are a lot of el cheapo JUNK IMITATION 
connectors and connector adapters sold to hams, both by ham dealers 
and at hamfests. I've traced a half dozen problems like this to 
these garbage connectors. I've learned the hard way -- it's name 
brand stuff (like Amphenol) or nothing!  

That doesn't mean that you might not have encountered a badly 
crimped or badly soldered connector. But my problems were definitely 
bad barrels and adapters (for example, N to UHF, N to BNC, BNC to 
UHF, Tees, elbows).
