the tower with the antenna on top.

tower
(assuming a suitable safety device at the top of the ladder to keep it from
slipping to the side) in order to attach or work on the antenna/mast? Tower
would be in the fully cranked down state, of course.
I've used an extension ladder but I had a small piece - 8-10" - of
aluminum with a notch in it (pointing towards the tower) that was bolted to the
top rung. It was self-centering on the tower and didn't slip. Of course it
needs to be tied in for safety.
Another approach is to lean the ladder vertically directly up to the
tower. Tie it in as far up as you can reach. Climb up 5-6' and tie it in again.
(I put my positioning belt all the way around the ladder and tower once I
start climbing the ladder.) Repeat twice more and the whole ladder will be
secured to the tower. Then it's an easy and safe way to get up to the top of the
tower.
