The original Appendix F that this one replaces, apparently was adopted in
Dec. 2000 and became effective Jan. 1, 2001.  You can find the original one,
currently in effect, at

It would appear that the NEW, PROPOSED REPLACEMENT was written by editing
the Microsoft Word document of that original Appendix F, but the person who
did this didn't bother to update the footer at the bottom of every page with
the incorrect and misleading dates on it.
