I like the LM470 base mount. They weld 19" X 3" X 1/2" ears to the #10 
rerod coming up out of the concrete.  IIRC these have 7, 3/4" holes to 
hold the base and raising fixture.  Once tthe concrete has cured the 
base is mounted, then the bottom of the tower is pinned to those ears. a 
hand winch easily raises the tower to vertical where hinge pins are 
inserted at the top of the base and the tower is bolted in place.   It 
can be cranked up, or the base pins removed so the tower can be tilted 
over to install, or work on antennas.
