The balun looks like a real old Hygain. I've seen a few over the years. I think that it is basically a bunch of turns of RG-8 inside a housing.

Hy-gain? Yes.
TH-3? Mine had molded traps (same color as the insulators) in all
 three 
elements.
They used those insulators in a number of antennas.
