Looks like way too much weight for the crane jib, the crane operator definitely was at fault because the LMI wouldn't have allowed him to do the pick if everything was set up correctly. 
Load Moment Indicator - it's a computer that all newer cranes have. The operator has to program the crane configuration into it - outriggers extended or half way, number of parts of winch line, load block or headache ball - the computer knows the weights, jib configuration, etc. The computer knows the boom angle and boom length , and calculates the weight and the load radius - the amount of leverage from the boom. 
The LMI won't allow you to get into a situation where the boom will fail or the crane will tip over if it's set up properly. 
Looking at the video, the crane is probably way underrated for the pick they did.

I happen to be in Luzerne County PA right now, Kingston to be precise, at
the in-laws.  That video has made its rounds through various emails, mailing
lists, web sites, etc..  I don't think it actually happened here in Luzerne
County...I probably would have heard about it when it happened.

To me it looks like a tower being taken down, not being put up, but hard to
say for sure.
