We put a 40-2CD for ARRL DX two weeks ago. The antenna had been in storage a few years and we found that a loading coil on one of the elementrs had gone open circuit. Application of some emery paper and re-tightening the screws sorted it out. Only 2 hours and a removed balun wasted, hi.

We use a 40-2CD in Africa.  It works perfectly every year, as long as it is built correctly!

The usual SNAFU are the screws connecting the feed point thru the centre insulator.  It is possible to put the screws in the wrong way around so that there is no connection to the aluminium.  It sounds like yours may have a bad connection on the braid side?

Mine at home has a VSWR like you descibe when one of the reflector coils is open circuit.  The coil connections get water under the heatshrink and corrode.  They need re-doing every 2 years or so.
