I guess there are many reasons why this happens:

1. A station comes on having not read the rules and sends his own 
postcode to everybody he works. Maybe he only works two stations - 
station A works him and then works you, passing on his code. Then 
said station calls and works you, passing along his own code again..

2. Station totally misunderstands the rules, and thinks he has to 
repeat the code you sent him as a confirmation for a good QSO...

And others.

These sorts of stations probably don't work very many people and 
probably never send in a log anyway so hard to trace what actually 
happened.
