Hi Ian

Yes, Page 1 of the licence plus copy of a Utility bill / driving licence
verifying the applicant's address - that will suffice.

But before doing that I recommend sending an e-mail to ARRL telling them
what you would like to do AND enclose a paper copy of that e-mail with the
above documentation.

So, to start with, try sending an e-mail to Bill Moore NC1L at ARRL HQ; he
has always been very helpful in dealing with this and other ARRL matters -
DXCC at ARRL.ORG will do the trick; he will usually pass same e-mail to a
colleague who will respond in quick time.  This in turn gives you a name
and call to whom you can send the correspondence.

Its a seamless exercise but as with any dealings with any large
organisation a named person with whom to conduct correspondence smooths
the path.
