It could be.
Imagine a "Sunday driver" who comes on to give a few points away. He
isn't very good at using logging program, but he wants to submit a
checklog because that will help the adjudicators.
He's in no hurry, and he often tidies up the computer log AFTER the QSO
is over.
He has worked 91 stations so far...
He has just found GM3POI who will be his #92. He calls, you go back to
him and he logs your serial number.
So far, so good. But now:
He notices that he mistyped your callsign as GM34POI (fat finger
trouble) so he decides to wipe the call field and type the whole thing
again. He doesn't notice that the serial number goes back to 91...
But to avoid keeping you waiting, he hits F2 and the logging program
sends 599 91 from his previous QSO.
The QSO is over, so in his own slow time he re-types your call. The
serial number now increments to 92 but he probably doesn't even notice.
He logs the QSO without sending anything more.
So now he has QSO #92 in his log, and GM3POI will be penalized for
having copied 91.
There are other variants of this scenario, but they all seem to revolve
around sending the previous serial number because the call frame is
empty at that moment. Clearly this doesn't happen routinely, but it
certainly CAN happen as I described.
