After checking Directive Systems web page I have found the antenna to be the new and improved version of a DSFO144-12. They changed the driven element design in 2019. What would we do without the internet? Mike

It might be an M^2 2M12LB. If the feed and balun connections are 
hardwired it would be an old version of that antenna. The newer version 
has a machined block to which the T sections, balun, and feed connect.
