[Amps] HV Interlocks on remote PS

With the discussion of HV wire between remote power supplies and the 
amp, I'm wondering how many actually use an interlock to shut off the HV 
in case the ground or HV wire become disconnected.  If any do, what 
method is used?

With the HV being in its own cable this can get a bit tricky.

In Industry we had to go through a good many hoops to make sure the 
power could not be turned on if the HV cable or ground were disconnected.
