I suggest the ATC series of flat strip multilayer ceramic capacitors like these:

I'm working on a high power amplifier design using high voltage
switching MOSFETs. It all looks fine in LTspice (doesn't it always?)
until I look at the PSU decoupling requirements. I need something of the
order of 100nF at 350V with a ripple current rating of 4.5A RMS at up to
10MHz.

Do such things exist? What sort of construction should I be looking for?
