One approach I've used with the non-IDing DX station is as follows:
DX: TU   finishing another QSO
ME: W2UP
DX: KC1XX 59936
KC1XX: 59905 CL?
DX: TU 
ME: W2UP
DX: W2UP 59936
ME: CL?
DX: W2UP 59936
ME: UR CL?

DX scenario 1: ZD8Z
ME: 59905
or
DX scenario 2: TU 

I request the call before giving the report, hopefully to get Mr. DX-
200+/hr-legendinhisownmind to ID.  In scenario 1, he IDs.  He may be 
unhappy how he got slowed down, but he did it to himself.  In 
scenario 2, he still refuses to ID, and if he logged my call, he 
loses the QSO + penalty.  Sceanrio 2 is more likely in CQWW where 
there really is no exchange to copy.  In ARRL DX he needs my state, 
so has to copy an exchange.  I've had both happen...

Please note, I used ZD8Z for illustrative purposes only.  I'm not 
implying he operates this way.
