A very easy explanation.
The M/S winner in 2016 CQWW DX CW - CN2AA - would have been in 3rd
place in M2X and 2nd place in MM if they had entered those categories.
From what I can tell they would run on a band and have a 2nd radio
working people on the same band. (i.e interleaving QSO's) They had a 3rd
radio which would work multipliers on a 2nd band.
Here are some excepts of the CN2AA 2016 CQWW DX CW log that are
available in the public logs sections of the CQWW web page.
Running on 3514 , interleaving QSO's on 80 meters, Multiplier radio on
40 meters.

I can tell you from operating CQWW DX CW in the M2X category in the Caribbean
for the last 13 years it's hard to stop during a pileup to work multipliers
when you have really good rate going on two bands at the same time. It wouldn't
be unusual for a M/S station to have more multipliers than a M2X. (or even a
MM) Plus the proximity of Morocco to Europe would make many more multipliers
available to them on 160/80 meters as compared to someone in the Caribbean.
When all the EU stations are working each other on 160/80 meters it's hard to
get their attention from the Caribbean.
